Please read these Terms of Service ("Terms") carefully before accessing or using our app. By using My Paperwork, you agree to these Terms. If you do not agree to these Terms, please do not use the app.
1. Overview of Services
My Paperwork provides:
- Access to booking lab testing for sexual health through our partner, Vital (https://www.tryvital.com/labs).
- Delivery of digital health test results directly within the app.
- Features to share your health results with others at your discretion.
- Dating and matching services to connect with other users in your local area.
2. Eligibility
- You must be at least 18 years of age to use My Paperwork.
- By using the app, you confirm that you meet this age requirement and are fully able to comply with these Terms.
3. Health Information and Results
- Booking and Testing: Lab testing is provided via Vital. By booking tests, you agree to comply with their terms and conditions.
- Results Delivery: Your health test results will be made available digitally in the app. It is your sole responsibility to review and interpret the information.
- Sharing Results: You may share your test results with other users or third parties at your discretion. My Paperwork is not responsible for how or with whom you share your results.
- Telehealth Consent: By using My Paperwork for lab testing or consultations facilitated by Vital and its affiliates (including Openloop Healthcare Partners), you consent to telehealth services, which may include videoconferencing, telephonic, or asynchronous technology. You understand and agree to the following:some text
- Telehealth providers will not be physically present during the consultation.
- Risks include interruptions, delays, unauthorized access, and technical difficulties.
- Either you or the provider may discontinue the telehealth session if technical issues arise.
- You have the option to seek in-person care instead of using telehealth services.
- You agree to undertake telehealth consultations in a private location to protect your confidentiality.
- In case of emergencies, the provider may direct you to emergency medical services (e.g., an emergency room).
By using the app, you confirm that you have read, understood, and agree to these telehealth terms.
4. Dating and Matching Services
My Paperwork offers dating features that allow you to:
- Match with other users based on your preferences and location.
- Communicate and engage with matches via the app.
Important Note: My Paperwork does not verify user identities or screen users. Use caution and your best judgment when engaging with others, meeting in person, or sharing personal information.
5. Privacy and Data Protection
- No Sale of Data: Your personal data, including health information, will never be sold to third parties.
- Partnership with Vital: By booking lab tests, your information may be shared with Vital for the purpose of fulfilling your lab testing requests.

6. User Responsibilities
By using My Paperwork, you agree to:
- Provide accurate and truthful information during registration and testing.
- Use the app lawfully and responsibly.
- Avoid sharing any sensitive or personal information with others unless you trust them.
You further agree that My Paperwork is not responsible for:
- The misuse of your shared test results by other users or third parties.
- Any interactions, meetups, or communication with other users outside of the app.
7. Prohibited Activities
You may not:
- Use the app for illegal or unauthorized purposes.
- Upload or share false, misleading, or harmful information.
- Engage in harassment, abuse, or any harmful behavior towards other users.
- Attempt to reverse engineer, hack, or disrupt app services.
Violation of these Terms may result in the suspension or termination of your account.
8. Disclaimers
- My Paperwork is provided on an "as is" and "as available" basis.
- We make no guarantees regarding the accuracy of health results or the quality of interactions with other users.
- My Paperwork is not a medical provider and does not offer medical advice. For medical concerns, please consult a qualified healthcare professional.
- Lab testing services are fulfilled by Vital, and My Paperwork is not liable for any issues or errors with lab results.
- Telehealth services facilitated by Vital and its affiliates are subject to the terms outlined in the Telehealth Consent section.
- Patients residing in New York and New Jersey have the right, under their respective state patient billing laws, to request an itemized price list from My Paper Work for their laboratory tests. Requests can be directed to support@mypaperworkapp.com for further assistance.
9. Limitation of Liability
To the fullest extent permitted by law, My Paperwork and its affiliates will not be liable for:
- Any direct, indirect, incidental, or consequential damages resulting from your use of the app.
- Loss, misuse, or unauthorized access to your test results.
- Actions or behaviors of other users, including any harm that arises from meetups or communications.
10. Termination
We may suspend or terminate your access to My Paperwork at any time, without notice, for violations of these Terms or if required by law.
11. Governing Law
These Terms are governed by and construed in accordance with the laws of North Carolina. Any disputes will be resolved in the courts of North Carolina.
12. Modifications to the Terms
We reserve the right to modify or update these Terms at any time. Continued use of the app after changes means you accept the updated Terms.
